Job description
HopeHealth is seeking a Hospice Admissions RN to provide skilled assessments and admissions for patients eligible for hospice services in home and long-term care settings. This part-time role is 32 hours per week with 1:00pm–9:30pm shifts and rotating weekend commitments.
You will collaborate with an interdisciplinary team, document in EMR, ensure payer information accuracy, and develop individualized care plans. A $2000 sign-on bonus is offered.
