Job description
Pathways Home Health & Hospice in Sunnyvale is seeking an experienced Hospice Admissions Visit Nurse (RN) to evaluate and admit patients to Hospice. You will document patient status, coordinate with the care team, and ensure timely care planning. This role is full-time with a 40-hour week and operates in the Bay Area’s hospice setting.
Requires CA RN license, CPR, and a valid driver’s license; BSN preferred. Sign-on bonus of $9,000 with conditions. Shifts available Sun-Thu or Tue-Sat, 2pm-10pm.
