Job description
Addus Family Company seeks a compassionate Hospice Admissions RN to coordinate and complete hospice admissions for patients in their homes. This field-based role requires performing initial assessments, verifying eligibility, obtaining consents, and creating individual plans of care with physicians and the interdisciplinary team.
You will educate patients and families on hospice philosophy, services, and symptom management, ensuring a smooth transition of care.
