Job description
Transitions Care is a leading hospice organization providing 24/7 home-based services to patients and families. As a Hospice Nurse, you will support the IDG team, assess patient needs, deliver care, and report to the case management team while ensuring timely fulfillment of patient needs.
Local travel is required. The role emphasizes professional nursing care within scope of practice, coordination with physicians, and ongoing education to maintain high standards of care.
