Job description
Addus Family Company is seeking a compassionate Hospice Admissions RN to coordinate admissions for home-based patients, ensuring a smooth transition of care.
In this field-based role, you will conduct initial assessments, verify eligibility, obtain consents, and develop individualized care plans with physicians and the interdisciplinary team.
You will educate patients and families on hospice philosophy, services, and symptom management.
