Job description
Haven Hospice in Palatka, FL is seeking a compassionate Hospice Admissions RN to join our team. You will visit patients in homes, hospitals, and facilities to assess eligibility, discuss goals, and initiate services with the interdisciplinary team.
Graduate nurses with state licensure and hospice experience preferred; drivers with reliable insured vehicles are required. We offer 1:1 patient care, supportive colleagues, and flexible scheduling in a growing organization.
