nHospice Aide

nFull-Time

nAs a Hospice Aide (CNA) you'll contribute to our success in the following ways:

nnnProvide skilled nursing care according to the individualized Plan of Care, physician orders, and direction of the RN.

n

nnMonitor patient condition and promptly report changes, concerns, or safety issues to the RN Case Manager, physician, and care team.

n

nnCollaborate with the RN Case Manager to address patient needs and recommend updates to the Plan of Care when appropriate.

n

nnEducate patients and caregivers on self-care techniques to support safety, independence, and overall health.

n

nnDocument medication regimens and maintain accurate, up-to-date medication profiles.

n

nnHelp prevent avoidable hospitalizations and emergency room visits through proactive, coordinated patient care.

n

nnProvide compassionate, high-quality care while maintaining positive communication with patients, families, physicians, and the care team.

n

n nWhat is Required?

nnnHigh School Diploma or GED equivalent required

n

nn1 year of experience as a Hospice Aide or Nursing Assistant is required

n

nnState-established or other Hospice Aide training program that meets the requirements of 42 CFR §484.36 and a competency evaluation program

n

nnAbility to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

n

nnMust possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

n

nnExperience in a home health environment is preferred
