We are dedicated to making a difference in the lives of employees, offering the opportunity to be associated with caring teammates, and creating positive contributions in each community we serve.What you must have:High school diploma preferred or GEDMust have an unencumbered certification according to the applicable nurse aide registryLicensure: current CNA licenseAt least one year experience as a hospice aide or nursing assistant in a hospital, nursing home, home health agency, hospice or long term care facilityDemonstrates interest in the welfare of ill and elderly; a heart for providing compassionate careHigh integrity, including maintenance of confidential informationEMR experience a plusMust have current driver's license, reliable transportation, and provide ongoing valid and current auto liability insuranceFull time employee benefitsCompetitive payMedical, dental & vision insurancePaid time offPaid holidays401k with up to 4% employer matchingTuition reimbursementCompany car for qualifying individualsMileage reimbursementWhat you will do:Provide direct patient care to patient under direction of the RN and according to the hospice aide/homemaker plan of care.Provide necessary skills to assist the patient with safe transfers and ambulation per agency policyProvide necessary skills to appropriately report/communicate changes and document pertinent information and care provided to patient to ensure continuity of care.Practice accepted infection control principles.Promote positive, supportive, respectful communication to patient/family and other employeesTimely submission of all documentationPerformance of other duties as requiredEnsure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and proceduresIt is this agency's policy to provide equal employment opportunities without regard to age, race, color, religion, military status, gender preference, sex, marital status, national origin or disability.
