nThe Home Care/Hospice Aide delivers high quality patient care in the home environment under the direction of the RN Case Manager. The Home Care/Hospice Aide provides personal care, homemaker services, and companionship to patients and ensures that high standards of care are met at all times.

nResponsibilities
nnnAssist patients with daily living activities including bathing, dressing, oral hygiene, and mobility
n
nnProvide light housekeeping, meal preparation, and companionship
n
nnSupport medication administration and simple procedures under RN guidance
n
nnObserve and report changes in patient condition or home environment to the RN Case Manager
n
nnMaintain accurate documentation and ensure compliance with care plans and regulatory standards
n
nnParticipate in interdisciplinary team meetings, in-services, and quality improvement initiatives
n
nnBuild strong, compassionate relationships with patients, families, and caregivers while maintaining professional boundaries
n
n nQualifications
nEducation:
nnnCompletion of a 120-hour Nursing Assistant course .
n
nnHigh School Diploma/GED High School diploma or GED
n
n nExperience:
nnnMust be at least 18 years of age in accordance with regulatory requirements Required .
n
nnCompleting of a Nursing Assistant Training Program of a minimum of 75 hours with no continuous period of 24 months without compensation for Home Care or Hospice Aide/CAN work after completion
n
nnRequired 2 years 2-5 years of home care or hospice experience
n
n nLicense(s)/Certification(s):
nnnValid Driver’s License (IA, IL, or WI) – Required upon hire
n
nnBasic Life Support (BLS) Certification – Maintain current certification through the American Heart Association (Required upon hire)
n
nnCertified Nursing Assistant (CNA) – State of Iowa (IADIA) or Illinois (IDPH) preferred
n
nnChild Abuse Mandatory Reporter Training – Iowa or Illinois, required within 90 days of hire
n
nnAdult Dependent Abuse Mandatory Reporter Training – Iowa or Illinois, required within 90 days of hire
n
