Evaluates patients referred for Hospice and /or Palliative Care services.
Manages and facilitates the transition of patients in hospice or palliative care in accordance with applicable law, regulations and agency policies and procedures.
Provides clinical support, leadership and oversight for growth and development of community and facility based hospice programs
Duties include planning, delivering customer service, development for service
area expansion, coordination of internal and external clinical marketing efforts, community outreach, education, and clinical service/support for hospice patient/family direct care
Education:
Graduate from an accredited School of Nursing; BSN preferred.
Experience A minimum of two years’ experience in either Community Health. xzgxqkn Hospice or Oncology nursing preferred.
