Job description
Allina Health is seeking an experienced Hospice Case Manager (RN) in Minneapolis, MN. This 1.0 FTE position offers 8-hour day shifts with every 4th weekend and travel eligibility with mileage reimbursement.
Responsibilities include comprehensive patient/family needs assessments, care planning, and coordination across disciplines to ensure safe, high-quality end-of-life care. Licensure in MN or WI is required on hire; BLS/CHPN may be preferred.
