Job description
Amedisys is seeking an RN Case Manager to join our home health and hospice care teams. You will perform patient assessments, develop and implement care plans, and collaborate with the care team to ensure high-quality, patient-centered care in the home setting.
You'll also coordinate referrals, maintain care records, and provide education to patients and families. On-call responsibilities are included as part of the role.
