Job description
UVM Health - Home Health & Hospice is seeking a Case Manager Registered Nurse to join their Community Hospice team in Colchester, Vermont. In this in-person role, you will provide skilled nursing care, education, and create supportive environments for patients and their families.
Ideal candidates will have at least 2 years of experience in home health, home hospice, or medical-surgical settings, along with a current RN licensure in Vermont. The position includes a $10,000 sign-on bonus, a comprehensive rewards package, and a focus on professional growth.
