Job description
Amedisys Home Health is seeking a Registered Nurse case manager for hospice care in the Triadelphia area. This full-time 1st shift role focuses on guiding patients and families to receive care at home.
You will perform patient assessments, develop and monitor care plans, and coordinate with the care team to ensure comfort, pain management, and timely referrals. A valid RN license and CPR certification are required.
