Job description
VitalCaring in the United States is seeking an experienced Clinical Care Manager (RN) – Hospice to guide patients and families with compassion, lead the plan of care, and coordinate a multidisciplinary team to deliver dignified end-of-life care.
As the hub for referrals, providers, and care teams, you will ensure timely documentation, adherence to regulations, and continuous assessment of patient needs while supporting colleagues with strong clinical judgment.
