Job description
VitalCaring is seeking an experienced Clinical Care Manager (RN) to guide hospice care, coordinate interdisciplinary teams, and ensure comfort and dignity for patients and families. This full-time role emphasizes collaboration with physicians, social workers, chaplains, and aides to deliver seamless, compassionate end-of-life care.
You will lead plans of care, monitor changes in condition, and support admissions and timely referrals from Intake.
