Job description
Haven Hospice in Saint Augustine, FL, is seeking a compassionate Hospice Clinical Specialist to provide clinical oversight, documentation management, and workflow coordination from Start of Care through discharge.
You will collaborate with interdisciplinary teams, ensure timely, compliant documentation, coordinate patient visits, and support staff development while delivering high-quality end-of-life care in patients' homes and community settings.
