Job description
The Care Team Hospice in Decatur, IL is seeking an experienced Registered Nurse to step into clinical leadership as Clinical Team Leader (RN). This full-time role partners with the Clinical Manager and Administrator to support daily operations, guide nurses, aides, social workers, and chaplains, and ensure compassionate hospice care.
You will coach clinicians, strengthen documentation and care planning, coordinate staffing, review care plans, and monitor quality through audits and outcomes.
