We are a fast-growing, high-acuity hospice organization committed to delivering exceptional end-of-life care with integrity, compassion, and clinical excellence. Our team serves a diverse patient population and maintains an average daily census (ADC) exceeding 100 patients across multiple service areas. We are seeking a seasoned clinical leader to join our executive team and oversee all patient-care operations.
Position Summary
The Director of Patient Care Services (DPCS) is responsible for the overall direction, coordination, and supervision of hospice clinical services. This role ensures regulatory compliance, clinical quality, and operational efficiency across all disciplines. The ideal candidate brings strong leadership capabilities, extensive hospice knowledge, and experience managing a large-scale hospice program. Previous Executive Director, Administrator, or multi-site leadership experience is strongly preferred.
Key Responsibilities
Provide strategic leadership and daily oversight of all clinical teams, including RN Case Managers, LVNs, MSWs, Chaplains, CNAs, and IDG processes.
Ensure compliance with CMS, CDPH, Joint Commission (if applicable), and hospice Conditions of Participation.
Oversee patient care delivery for an ADC of 100+ patients, ensuring quality outcomes and timely interventions.
Lead IDG / IDT meetings, clinical audits, quality initiatives, and performance improvement activities (QAPI).
Maintain and enforce visit frequencies, care plans, documentation standards, and regulatory timelines (FTF, NOE / NOC, CTIs, recertifications).
Partner with Executive Director / Administrator to drive operational performance, patient outcomes, staffing efficiency, and fiscal stewardship.
Recruit, train, mentor, and evaluate clinical staff to uphold a high standard of service and professionalism.
Serve as the primary clinical liaison with physicians, hospitals, families, and community partners.
Participate in on-call coverage, crisis management, and escalated patient-care situations as needed.
Qualifications
Active RN license in California (required).
Minimum 35 years hospice leadership experience.
Experience managing a hospice agency with 100+ average daily census (required).
Previous Executive Director, Administrator, Advanced practice licenses or multi-branch leadership experience strongly preferred and will be compensated accordingly.
In-depth knowledge of Medicare hospice regulations, COPs, IDG structure, and charting / documentation workflows.
Demonstrated success in team leadership, conflict resolution, and cultivating a strong clinical culture.
Ability to thrive in a fast-paced environment with high acuity and significant operational volume.
Strong communication, organizational, and decision-making skills.
