Job description
Transitions Hospice - Illinois is seeking an experienced RN to join our Extended Care Team (ECT) as-needed on a PRN basis. The role focuses on providing flexible, high-quality nursing support across Bloomington, Pontiac, Lincoln and surrounding areas.
You will assess, treat, and communicate patient needs with speed and clinical precision, coordinate with care teams, administer meds per orders, and maintain meticulous documentation while traveling locally to patient homes.
