Job description

Halifax Health-Hospice seeks a Registered Nurse – Hospice Field Case Manager to coordinate and implement the plan of care for assigned patients across hospice settings, ensuring the best possible quality of life for each patient.

Responsibilities include patient/family education, regulatory compliance, comprehensive assessments, symptom management, and developing the interdisciplinary plan of care. Must hold a Florida RN license and a valid driver's license; hospice experience is preferred.
