Job description
MJHS Hospice and Palliative Care in Garden City, NY seeks a Field RN Case Manager to visit patients in community and facility settings, perform comprehensive assessments, and document visits electronically. You will develop and implement nursing plans of care and provide recommendations based on patient needs.
Qualifications include an Associate's/Diploma in Nursing (BSN preferred), NYS RN license, NYS Driver's license, and 2+ years in med/surg, community health, or oncology nursing.
