Job description
Metropolitan Jewish Health System is seeking an RN Case Manager to perform physical and psychosocial assessments for end-of-life patients and develop a comprehensive nursing care plan. You will supervise home health aides and care staff, coordinating orders with physicians to adjust care plans as needed.
The role requires NY RN licensure, a valid NYS driver's license, and experience in an interdisciplinary team. On-call duties 2–3 times monthly and some holidays are included.
