Job description
Transitions Care in the United States seeks a compassionate Registered Nurse to provide hospice visit care in patients' homes and facilities. You will deliver skilled nursing assessments, manage symptoms, administer medications, and implement individualized care plans focused on comfort and quality of life.
Educate and support patients and families, coordinate closely with an interdisciplinary hospice team, and ensure timely documentation.
