Job description
Affinity Health Management in Ohio is seeking a Registered Nurse for hospice care. The RN will plan, organize, and direct home-based hospice services, drawing on community resources to support patients and families.
You will perform assessments, develop and revise care plans, administer medications, educate families, and coordinate with the interdisciplinary team. Previous hospice/public health experience is preferred; licensure in the state is required.
