Job description
Greater Baltimore Medical Center (GBMC) seeks a Hospice Home Care Nurse to identify patient and family needs and provide comprehensive nursing and supportive care for terminally ill patients and families in home or facility settings.
Responsibilities include completing assessments, developing care plans with the Interdisciplinary Group, delivering direct nursing care, and supervising HHAs, while coordinating with the Clinical Manager and primary physicians to ensure optimal end‑of‑life care.
