Job description
Hartford HealthCare at Home is seeking a Hospice Visiting Nurse (RN) to support patients and families through the end-of-life journey. You will perform comprehensive assessments and set goals for symptom management within an interdisciplinary team.
Successful candidates will have CT RN license, at least one year of RN experience (homecare or hospice preferred), and reliable transportation. Computer proficiency and professional communication are essential to document skilled services.
