Job description
The Community Hospice is seeking a Per Diem Registered Nurse to join our Hospice Home Care Team serving patients and families throughout Schenectady and Albany Counties. Availability during the workweek, 8:00 a.m.
to 4:30 p.m., with no weekends or holidays, is required for this per diem position. You will provide compassionate, home-based nursing care and collaborate with an experienced interdisciplinary team to coordinate individualized plans of care that honor each patient's goals and quality
