Job description
Yale NewHaven Health is seeking an Admitting Nurse to join our Home Care and Hospice team. You will assess patients and families, develop care plans, and coordinate with providers to ensure safe, high-quality visits.
You will educate families about hospice philosophy, CMS data, and care milestones while managing documentation within 24 hours of visits. Travel and after-hours coverage may be required in this role.
