Job description
Three Oaks Hospice is seeking a Registered Nurse to provide skilled nursing care for patients in their home or residence. You will coordinate care with the interdisciplinary team and family, implement the plan of care, and document patient progress.
The role includes on-call responsibilities, patient/family education, and ensuring compassionate, compliant end-of-life care in a supportive hospice setting.
