Job description
Quality Home Health, Hospice, and Palliative Care in Salt Lake City seeks a part-time Hospice RN Case Manager to coordinate and deliver nursing care in patients' homes across the continuum of home health and hospice services.
You will perform assessments, develop care plans, supervise a care team, educate patients and families, and ensure regulatory compliance while supporting quality of life and dignity.
