Job description
Community Hospice and Palliative Care in Florida is seeking an RN Case Manager to plan and manage nursing care for an assigned caseload across home and long-term care settings, coordinating with the interdisciplinary team to meet patient- and family-centered goals.
The role involves educating patients and caregivers, coordinating care across sites, and participating in IDG meetings, with flexibility to work across care settings and extend hours as needed.
