Job description
Role Overview:
The Hospice Homecare Case Manager Registered Nurse provides compassionate, patient-centered care in a home setting, coordinating services to support patients and their families through end-of-life care.
Key Responsibilities:
Assess patient needs and develop individualized care plans
Coordinate with multidisciplinary teams to ensure comprehensive care delivery
Educate patients and families about disease management and care options
Monitor patient progress and adjust care plans accordingly
Document care activities accurately and maintain compliance with healthcare regulations
Qualifications & Skills:
Registered Nurse (RN) license required
Associate Degree in Nursing or higher
Minimum of 1 year of relevant experience
Basic Cardiac Life Support (BCLS) certification within 30 days of hire
Valid Driver License within 30 days of employment
Strong communication, organizational, and compassionate skills
