Job description
Northwestern Medicine is seeking a Registered Nurse – Flex HHC in Chicago, IL. The nurse provides direct patient care across the admission–discharge continuum, ensuring age-appropriate, evidence-based plans of care and effective patient/family education.
The role emphasizes coordination with the health care team, documentation of care plans, and ongoing reassessment of outcomes. Illinois RN licensure, BSN preferred, and a valid driver’s license are required.
