Job description
Providence at Home with Compassus in Santa Rosa is seeking an Intake assessment LVN with home health experience. The role centers on accepting referrals via phone and fax and coordinating with clinicians to ensure timely admissions.
Qualified candidates have California LVN licensure and at least one year of acute care and home health experience. The position is in-office with a 8:00am–4:30pm PST schedule and a comprehensive benefits package.
