Job description
Coastal Home Health & Hospice is seeking a Nurse Practitioner to provide comprehensive hospice services under a collaborating physician. You will assess patients, establish care plans, prescribe treatments, and coordinate with families and the hospice interdiscliplinary team in patients' homes and communities.
The role requires an NP with a Master's degree, DEA license, and hospice experience, plus strong communication and documentation skills.
