Job description
Shalom Hospice is seeking a Nurse Practitioner to provide comprehensive hospice services under physician supervision. Responsibilities include patient evaluation, plan of care development, medication management, and coordination with families and the hospice team in home-based settings.
The NP will perform face-to-face assessments, determine hospice eligibility, and collaborate with the interdisciplinary team to ensure quality, compliant care for patients with life-limiting illnesses.
