Job description
UnitedHealth Group’s Community Loving Care Hospice, part of LHC Group, is seeking an RN Case Manager to oversee the Plan of Care for hospice patients and to collaborate with the IDG and other clinicians to promote holistic well-being.
Responsibilities include initial eligibility evaluation within 48 hours, ongoing reassessment, care coordination, and thorough documentation. A sign-on bonus is offered for external full-time candidates who join the team.
