Job description
Gentiva Hospice is seeking a skilled RN Case Manager to lead home-based hospice care, partnering with patients and families to implement individualized plans of care. You will conduct initial and ongoing assessments, deliver hands-on nursing care, and document care per policy across patients’ homes and allied settings.
