Job description
Affinity Health Management seeks a Registered Nurse to plan, organize, and direct hospice care in home and community settings. You will rely on nursing expertise to educate families and coordinate services with the interdisciplinary team.
You’ll perform comprehensive assessments, develop and revise care plans, administer medications, and provide patient and family education with empathy. Strong communication and case-management skills are essential.
