Masonicare Home Health & Hospice in Norwich, CT is seeking a Hospice Nurse Liaison for day shift (40 hrs/wk). The role involves evaluating and admitting patients to the GIP and routine hospice services, developing initial plans of care, and maintaining accurate documentation.
