Job description
Emerald Coast Hospice is seeking a dedicated RN Case Manager in Florida to lead patient care in home and community settings. You will coordinate with an interdisciplinary team to develop and update individualized plans of care, educate families, and ensure compassionate end-of-life support.
The role requires an active RN license, CPR, and a minimum of one year of RN experience, with hospice or palliative care background preferred.
