Job description
CONTINUUM CARE HOSPICE is seeking a Nurse Practitioner to provide comprehensive hospice services under a collaborating physician. You will perform history and physicals, assess patients, develop individualized care plans, prescribe medications, and coordinate with the hospice team to deliver home-based care.
