Job description
Optum's Deaconess Hospice unit is hiring a Hospice Nurse Practitioner to provide consultation in palliative and supportive care, following the referring physician's directions. Face-to-face visits with eligible hospice patients are performed within prescribed timeframes.
The role collaborates with the Hospice Medical Director and interdisciplinary team, coordinates the plan of care, educates patients and families, and maintains detailed clinical notes.
