On-Call Registered Nurse (7 On / 7 Off)After-hours support. The After Hours On-Call Registered Nurse provides skilled nursing support, symptom management, and crisis intervention to hospice patients receiving care in home and facility settings during evenings, weekends, and after-hours coverage while collaborating closely with the interdisciplinary team to ensure compassionate, quality care.Schedule, Hours & Territory• Dedicated on-call role with a rotating 7 days on / 7 days off schedule• Coverage hours are 4:00 PM to 8:30 AM• Tawas, MI to West Branch, MIWhat We Offer• Predictable 7 on / 7 off schedule• Mostly remote role with field support as needed• Strong clinical and leadership supportBenefits• Competitive Total Care Benefit Package, compensation, and mileage reimbursement per company policy• 401(k) with employer match• Health Savings Account eligibility with employer contribution up to $1,000 annually• Employee Assistance Program, discounts on gym memberships and wellness programs• Tuition reimbursement• PTO begins accruing upon hire, with full-time team members earning up to 3 weeks of PTO during their first year of employment and increasing accruals with tenure, plus seven paid holidays.Key Responsibilities• Triage patient and caregiver calls after hours• Provide clinical guidance and symptom management• Complete urgent in-person visits as needed• Coordinate care with interdisciplinary team members• Document all assessments, interventions, and communication accurately and timely in the EMR HCHB (Homecare Homebase)• Active Registered Nurse (RN) license• Hospice field experience preferred• Strong clinical judgment and decision-making skills• Experience using HCHB (Homecare Homebase) preferredField Role Requirements• Active driver’s license• Ability to pass a criminal background check• Ability to pass a Motor Vehicle Record (MVR) check• Current TB testing in accordance with agency policy• Current CPR certification as required for the role
