Job description
Affinity Health Management is seeking an On-Call Hospice Registered Nurse to respond to after-hours patient census and referrals, providing urgent clinical and psychosocial care, pain and symptom management, and coordination with the Hospice Medical Director and on-call administrator.
Responsibilities include after-hours assessments, documenting care, and collaborating with the care team to ensure continuity of care for patients and families in their homes.
