Job description
UnitedHealth Group's Northern Arizona Hospice in Cottonwood, AZ seeks a Hospice Nurse Practitioner Specialist to provide expert palliative care, symptom management, and supportive care for hospice patients under physician guidance.
You will conduct face-to-face visits, collaborate with the Hospice Medical Director and primary physicians, develop individualized care plans, educate patients and families, and maintain detailed clinical notes to document patient status and changes.
