Job description
Affinity Health Management seeks a Nurse Practitioner to provide hospice services under physician supervision. You will evaluate health history, perform physical assessments, diagnose conditions, and develop plans of care to manage terminal illness in home and community settings.
You will collaborate with the interdisciplinary team, educate patients and families, complete hospice eligibility reviews, and ensure compliance with Medicare/Medicaid guidelines.
