Job description
The Hospice/Palliative Certified Nurse Practitioner provides advanced clinical practice and consultation for Veterans in the Community Living Center neighborhood and palliative care for inpatients and outpatients at the VA. You will lead an interdisciplinary team, manage a patient panel with acute and chronic conditions, and ensure quality, timely care aligned with ICARE values.
