Job description
Gentiva is seeking a Patient Care Manager to lead clinical operations and coordinate care in our hospice team. Reporting to the Executive Director or Administrator, you will oversee patient care activities, manage visits, triage referrals, and ensure regulatory compliance across services.
You will mentor staff, facilitate IDG meetings, and drive performance improvements while maintaining high standards of patient care in a compassionate home-based setting.
